FDA Warning: Infection Risks From Contaminated Ultrasound Gel

The Food and Drug Administration is warning doctors, hospitals and clinics that contaminated ultrasound gel produced by a New Jersey company infected 16 cardiac patients and could pose serious risks to pregnant women and others who undergo ultrasound imaging and treatment.

The gel is used by radiologists, urologists, gastroenterologists, OB-GYNs, internists, nurses and ultrasound technicians for diagnostic ultrasound testing. Chiropractors and physical therapists use the gel for therapeutic ultrasound treatment of pain, inflammation and injuries.

The agency told the health professionals to stop using the gel because of contamination with two strains of bacteria. “Although Other-Sonic Generic Ultrasound Transmission Gel is not labeled as either sterile or non-sterile, it is NOT sterile,” the FDA cautioned.

The 16 heart patients became infected with the bacteria, Pseudomonas aeruginosa, while undergoing transesophageal ultrasound exams with Other-Sonic Generic Ultrasound Transmission Gel during heart valve replacement surgery at a single hospital, the FDA said. The product is made by Pharmaceutical Innovations Inc., of Newark, N.J., which bills itself on its website as “world leader in medical ultrasound.”

In February, an FDA analysis of product samples revealed ”significant amounts of Pseudomonas aeruginosa and Klebsiella oxytoca,” which the FDA said suggested that the contamination occurred “during the manufacturing process.”  Both types of bacteria can colonize the skin without causing any symptoms, although Pseudomonas also may cause skin eruptions, even on unbroken skin, the FDA warned. More concerning, however, is the potential of the bacteria to enter the body through ultrasound exams of the airway, lower digestive tract or a woman’s genital tract, where they could cause infection.

“Intact skin is a fairly good barrier to infection,” said Dr. Robert A. Winters, an infectious disease specialist in Santa Monica, Calif. He said that  fetal ultrasounds and ultrasounds of organs like the gall bladder, where the gel is applied to overlying skin, are of “lower concern” than more invasive ultrasounds where a probe is inserted into the body, such as during transesophageal imaging. “On the other hand, they’re using the probe, they’re moving it on the skin, they could be abrading the skin and potentially introducing germs into otherwise clean sites. There is probably some small added risk if you’re undergoing ultrasound with a contaminated gel product.”

But he noted, in the big scheme of infectious disease risks, “We all are interacting with lots of different bacteria all day long.”

If you’re concerned about possible exposure to a contaminated gel, “the best thing to do is go home and take a good shower with soap and water,” or make sure the people performing your ultrasound “pat the area with alcohol afterwards,” said Winters, chairman of the infection control division at Saint John’s Health Center in Santa Monica.

“Not every patient exposed to Pseudomonas aeruginosa and Klebsiella bacteria in Other-Sonic Generic Ultrasound Transmission Gel will develop colonization (the presence of bacteria at a site without any signs of infection) or infection, but the risk remains present,” the FDA alert said. It warned that biopsies can let bacteria get into tissues, causing an abscess or severe bloodstream infection called sepsis. If Klebsiella bacteria, which are common in the digestive tract, enter the lungs or spread to other tissues, they could lead to pneumonia, wound infection or bloodstream infections, the FDA said.

At the agency’s request, New Jersey health authorities embargoed all lots of the gel manufactured from June through December 2011. The U.S. Marshal Service has since seized those lots. Health professionals were urged to stop using 250 milliliter bottles and 5-liter dispensing containers with three lot numbers: 060111, 090111 and 120111. They were also asked to identify any patients who underwent ultrasound exams with gel from those lots.

“This ultrasound gel presented serious health risks to patients, particularly vulnerable ones,” Dara A. Corrigan, the FDA’s associate commissioner for regulatory affairs, said in a statement issued by the FDA. “Therefore, FDA, with the assistance of our state partner, is taking aggressive enforcement action to protect the public health.”

REVIEW:

As we expected, the GE Voluson 730 Expert BT08 functioned perfectly! There is no question as to why the Voluson 730 has been the staple ultrasound machine for most 3D/4D ultrasound studios for the past 6 plus years.

When we walked into the room with the Voluson 730 in it, it gave us mixed emotions. The first being, how do we give an honest review on a machine that we have grown to love over the past years, and the second being “o boy… here we go again.”

The GE Voluson 730 is beast, both in size and production value. The machine itself is large and cumbersome, it’s as though it was taken from the 1990′s. GE is has yet to do a full redesign on the actual 730 line. This leaves us, at first glance, with the feeling of using an old and outdated machine; this couldn’t be further from the truth. The GE Voluson 730 Expert BT08 has everything a child in a candy store could want.

The one item installed on this machine that makes us smile is SRI II. Yes, it greatly helps improve your 2D images, but WOW is all we can say about the 3D/4D. When you look at an image with SRI turned off and then turn it on, you will be blown away. GE has done such a great job with this.

Being that we have been using GE Voluson 730′s for over 7 years now, we had to do very little tweaking to get the images to be “perfect.” The GE has a very easy way of saving settings, as well as saving multiple setting and having them at your fingertips.

When testing this machine we had 10 different women come in. The women ranged from 15 weeks up to 35 weeks. They varied in body mass size, placenta location and racial demographics. We tried to get a good sample base so that we could test the machine on true-life situations.

We will describe the image quality based on gestation weeks.

15-20 weeks: This stage on the Voluson 730 is great! You are able to tweak the threshold and gain enough to let you see the baby in great detail. (Yes, at this stage, the baby doesn’t have much fat, but this machine helps “add” fat and layers to smooth out the baby.

20-24 weeks: The images were much better than 15-20 weeks.

25 plus weeks: This is the sweet spot for the Voluson 730 Expert. The images we got were great. Plenty of detail in the face. The downside though: it’s hard to work and fully get a placenta out of the way. If you take your time, and the baby is directly up against the placenta, you can still get some great images.

Overall 3D/4D image quality is great. You can get images from 15 plus weeks (earlier is possible, we just didn’t test it this time) After using the Voluson 730 again, we remember why we love them.

Ease of use:

Being that we have been using the Voluson 730 for years, it was very easy to use. The touch screen simplifies the process. Most everything can be changed or adjusted at either a touch of the touchscreen or a simple twist of  knob.

The machine is very forgiving. If you accidentally get into an application or change a setting, you can easily go back to your default settings.

Conclusion:

The GE Voluson 730 is a great machine, from start to finish. Do you pay a higher price for the GE name, possibly… Is it worth it, in my opinion… Under most circumstances yes. I am still a huge fan of the Voluson product line, from the original 1.07 to the pro and the expert models.

Multi-Beam Imaging

Multi-Beam Imaging is a technique involving creating several scan lines of images in one TX. The Multi-Beam technique is possible through digital beam-forming, and is a function that cannot be done with existing analog beam-forming methods.

Synthetic Aperture

Synthetic aperture control is a method for overcoming the limits of the physical channel by using software. As shown in the diagram, to create one scan line, the TX and RX are performed twice.

REVIEW:

Shocked… That was my first impression of the new SonoAce R7, and I’m happy to say this. The reason I was so shocked was based on my previous experiences with past models put out by Medison. In the past, the only time that I would touch a Medison was really by accident. (Remember that we primarily do 3D/4D ultrasound when taking this into account. As well as the fact that we are HUGE GE Voluson fans.) In the past Medison products were known for poor images, laggy refresh rates, an ugly huge box/housing, and are extremely difficult to tweak to get images that look half decent; well that is no longer the situation.

When you first walk into a room with the Samsung Medison SonoAce R7, you start to think that maybe someone is going to wheel the machine in later. The R7 is TINY, sleek, and plain out sexy! It has a beautiful thin 17-inch LCD monitor, sitting on top of a small perfectly organized keyboard and control panel. All of this sits on top of a slender and skinny box. The machine is a nice bright white and mellow gray in color with orange color accents. The control panel is set up much like all the new Samsung Medison products.

After the shock of seeing one of the first eye pleasing ultrasound machines was starting to wear off, our first patient entered the room (typical size mom, 30 weeks pregnant.) We decided to do the first scan with strictly the stock 2D and 3D/4D settings provided on the machine… The 2D looked absolutely grand… The 3D/4D was a completely different story! With close to 10 years of 3D/4D scanning under our belts, we had a hard time figuring out what the 3D image was of. We could obviously tell by looking at the split screen that we had an ideal 2D image but the 3D/4D image just wouldn’t produce correctly. We were about to give up on Medison again and throw in the towel. Then we figured we had enough invested in the machine that we should try to figure it out… AND BOY O BOY DID WE!!!

After changing several settings in both the 2D settings as well as in the 3D/4D settings we were amazed at the difference in image quality. It was like night and day. We continued to tweak and make changes, especially in the 3D/4D settings, and ended up with great photos.

One major highlight on this machine is the Dynamic MR, if it wasn’t for this function the images wouldn’t be nearly as crisp or spot on for 3D/4D. Most people assume that the Dynamic MR will help with the 2D mainly and to be honest we thought the same thing. We were so wrong. 3D/4D images without the Dynamic MR on lacked definition and the wow factor. The moment you turn it on, it’s like you are scanning on a completely different machine.

15-20 weeks: We were not impressed with the 3D/4D at this stage. It was much like older Medisons or even older GE products. The images were very boney and it was difficult to compensate. To be fair though, on most machines this stage doesn’t look great due to the lack of fat on the baby, but the Medison seemed to fair worse than some other machines.

20-24 weeks: The images were much better than 15-20 weeks, but still left some room for improvement.

25 plus weeks: WOW! This is where this machine is awesome. The SonoAce R7 produced some of the best images that we have ever seen! There was so much detail in the babies face, hands and feet. After seeing these images I hope other companies (cough cough… GE) can work on getting the same detail.

Overall 3D/4D image quality is good. The Samsung Medison products use a different rendering color scheme and texture then say GE does, but this is what allows the Medison to create such great 25 plus week images with so much detail.

Ease of use:

This is always a hard one to test. What we did to see how easy this machine is to learn and work on; is we brought in a nurse who had NO ultrasound experience at all. By her having no experience it allowed us to start from scratch without her having any preconceived notions from other machines.

We were able to teach her the basic functions of the machine, how to obtain a 2D side profile, and how to get basic 3D/4D ultrasounds in less than 2 days! This is impart due to the fact that we created new programs and setting so that she wouldn’t have to change very many settings, but she did great.

I would have to say from her experience on the machine as well as ours this is a very easy machine to use.

Conclusion:

All and all this is a great machine, especially since it is brand new and comes with a manufacturers’ warranty. The machine is small and sleek and it produces great images.

Things to think about… If you plan to do 3D/4D ultrasound; are you looking for a machine that produces great 2D images throughout the pregnancy and killer 3D/4D images later in the pregnancy (25 plus weeks)… If so, this machine maybe for you. If you want to and plan to do a lot of your 3D/4D earlier in the pregnancy (15-24 weeks) just keep in mind that this machine does produce boney images that are the perfect quality.

“When is the best time to come in for a 3D/4D ultrasound,” and “when is the optimal time to have our clients come in for an ultrasound,” are the two questions that I hear more than any other. And I give one simple answer to both my studio clients and my business clients, even though it’s worded differently, it has the same meaning. If I’m speaking to a perspective studio owner, or a studio owner, I tell them that the best time is ASAP. Have your clients do a 3D/4D scan when they do their gender determination (typically 15-19 weeks). After this, most clients get “hooked” and want to come back for more. If you are priced right and provide a great service you will often have your clients return around 21-24 weeks to see the growth changes, and then 28-32 weeks to get the optimal 3D/4D photos.

The same goes when you talk to the end customer. Seeing the baby in 3D/4D at 15 weeks is often “alien like” but it’s still their baby and parents will love and appreciate seeing their baby in this stage. At 15-19 weeks you are still able to see the baby’s complete body. As the baby grows you become limited on the amount of the baby that you can see. At 21-24 weeks the baby is just starting to form their fat and their full facial features. This is still a great time to see the growth progress of the lil one. When you get closer to 28-32 weeks the client is able to typically see their baby in the “typical” 3D images that we are all accustomed to seeing in magazines and online.

With all that being said, the mothers body type and baby positioning also plays a role into when you can obtain the “best” images. If a client is only going to come in once, typically the all around best time to come in will be 28-30 weeks. As the baby nears 30 plus weeks, the baby starts to run low on room and fluid, one runs the risk of having the baby “smashed” or without enough fluid in front of their face, and therefore you will be unable to obtain the optimal photos that most clients are accustomed too.
